Home Support Services - Home Care Packages Programme
Your Aged Care at Home is committed to promoting the dignity and quality of life of the elderly community and their family and carers. We offer a range of services under the Government's Home Care Packages (HCP) Programme, as well as private Home Care services.
Enjoy access to a variety of services including, domestic assistance, personal care and high care, wound care, transportation, occupational therapy, and much more. If you feel that your home is no longer adequate, talk with us about home modifications and assistive technologies.

If you wish to access the Government's Home Care Packages program you will need to begin by contacting "My Aged Care" to begin your initial assessment. If you are yet to apply for an assessment or need general information about Home Care Packages, please click on the link to My Aged Care here.

New Aged Care Quality Standards
Organisations providing Commonwealth subsidised aged care services are required to comply with the Aged Care Quality Standards (Quality Standards). Organisations will be assessed and must be able to provide evidence of their compliance with and performance against the Quality Standards from 1 July 2019.
The Quality Standards focus on outcomes for consumers and reflect the level of care and services the community can expect from organisations that provide Commonwealth subsidised aged care services.

Personal Services
Bathing, showering, grooming, dressing. Toileting, mobility transfers.
Activities of Daily Living
Assistance with impaired hearing, sight or speech.
Nutrition and Diet
Meal preparation and assistance with eating. Providing enteral feeding formula and equipment.
Continence Management
Assessment for/providing pads, commodes, bedpans, catheter and urinary drainage appliances. Assistance with continence aids and appliances and managing continence.
Management of skin integrity
Providing bandages, dressings and skin emollients.
Domestic Services
Mopping, vacuuming, washing dishes, cleaning bathrooms, making beds, dusting, and much more.
Laundry Services
General laundering of clothes and linen. Arranging dry cleaning services.
Gardening and lawn maintenence
Light gardening – weeding, pruning, lawn mowing, minor garden maintenance, clearing debris.
Medications Management
Assisting in managing and taking prescribed medications.
Transportation
Transportation to medical appointments, social events, venues, shopping.
Rehabilitation Support
Assistance to access rehab support.
Respite Care
Short-term or temporary care according to your needs.
Home Maintenence
General repairs and household maintenence.
Home Modifications
Hand rails, easy access taps, shower hoses, ramps, stair lifts
Social Activities/Support
Arranging social activities and providing/coordinating support to social events, companionship, assist in accessing the community.
Nursing/Clinical Care
Assisting in treating and monitoring medical conditions at home.
Allied Health Services
Speech Therapist, Podiatrist, Occupational Therapist or Physiotherapist
Hearing and Vision Services
Audiologist, Audiometrist, Ophthalmologist
Wound Care
Wound care management.
Access to other health and related services
Referral to health practitioners or other health service providers.
